Memory Care
With a growing and aging population, the number of people presenting in the early stages of cognitive change is increasing. There is a need to increase the capacity to access the standard and early assessment of dementia. The role of screening and timely assessment of the patients in the early stages of cognitive change is essential.
Dementia is a progressive disease and its effect on patients and families is preventable. there is a number of people undiagnosed and untreated properly living with early stages in dementia in the community who need proper care.
In Harbourside Medical Clinic we try to close this gap by timely comprehensive assessment and collaborative approach in primary geriatric care.
We work closely with family doctors and nurse practitioners with the goal of providing continuous care to patients and support families.
In the Memory Clinic, the patients will be assessed by professionals using standard cognitive tools.
The clinic will offer Brain Health exercises and classes for individuals and groups.

Primary Care Geriatric
Home visit
In this clinic, we offer collaborative care to frail older adults who are home-bound and require special care at home.
Our multidisciplinary team will assess the patients at home and communicate with other health care workers to build the proper care plan for the patient.
Our approach to care is patient-based and with respect to the patient and family's values.
